Prior notification is required for the following:

Durable Medical Equipment (DME) of more than $1,000 (DME of less than $1,000 will be covered without prior notification only if medically necessary.)

Hospice

Prosthetics

Behavioral Health - Inpatient Admission

Alcohol and Other Drug Abuse Hospital Admissions

Transplants
If the provider is In-Plan, the provider will obtain prior notification. If the provider is Out-of-Plan, prior notification is the member's responsibility. Prior notification for transplants is always the member's responsibility.
