Trilogy Logos and ID Cards

Trilogy has a few different product logos that may appear on member ID cards. These logos must appear on the front of the ID card. See examples of Trilogy logos and ID cards. Please use our Find a Provider page to find out if your organization is in network.

Modifier Pricing

Trilogy applies certain CPT Modifier pricing adjustments to claims usually consistent with industry standards. For a specific list of Modifiers included in Trilogy Pricing, please contact us.
